The Naturopathic Protocol for Weight Loss Medications
Over the past year, you have likely seen ads for GLP-1 medications (such as Ozempic, Mounjaro, and Rybelsus). These medications were first developed to treat Diabetes and have gained tremendous popularity for weight loss, having recently been approved by the FDA for treating obesity.
While these medications may be beneficial for some, they also come with their own risk of side effects including muscle loss and interference with the absorption of nutrients. How do these medications work and what is the naturopathic protocol for maintaining muscle?
The Science Behind GLP-1 Medications
GLP-1 medications are a class of medications primarily used to manage blood sugar levels in individuals with Type 2 Diabetes. GLP-1 medications mimic a hormone called GLP-1 which is released in the gut. GLP-1 tells the pancreas to make more insulin to help control sugar levels which slows down our digestion so we feel full. You may be familiar with some of the earliest versions of these medications, Exenatide and Liraglutide/Victoza. Ideally, these medications should be used to complement healthy lifestyle and dietary habits.
Types of GLP-1 Medications
Semaglutide (also known as Ozempic) is a great option for treating Diabetes. The weight loss version of Semaglutide is called Wegovy.
Did you Know? Semaglutide has been shown to reduce the risk of strokes, heart attacks, and cardiovascular death. Additionally, it may improve blood pressure and cholesterol levels.
Tirzepatide (also known as Mounjaro) is another example of a GLP-1. The weight loss version is called Zepound. This medication is a bit stronger than Semaglutide because it works on two receptors -both GLP-1 and glucose-dependent insulinotropic polypeptide (GIP).

Not only do these medications help treat Diabetes and obesity, but they can also be helpful for certain heart conditions. While it may seem like these medications are an easy fix for weight loss, there are a vast array of serious side effects reported.
Side Effects of GLP-1 Medications
Documented side effects include:
- Nausea/Loss of Appetite
- Reflux/Vomiting
- Diarrhea/Constipation
- Low Blood Sugar
- Gall stones
- Increased risk of Biliary Disease, Pancreatitis, Bowel Obstruction, Gastroparesis
GLP-1 Medications and Muscle Loss
Because GLP-1’s interfere with the absorption and utilization of essential vitamins and minerals in the body, they can potentially lead to nutrient depletions over time.
A side effect we see commonly is loss of muscle mass. When losing weight, it’s normal to lose some muscle mass. However, we want to minimize muscle loss and maximize fat loss. Preserving muscle is vital for maintaining weight loss long-term, preventing osteoporosis, and improving overall longevity. The quicker and more drastic the weight loss, the higher the risk of losing muscle mass.

- Strength training at least 3x per week
- Monitoring weight loss and ensuring you aren’t losing more than 1.5-2.5 pounds per week
If you are 65 years or older or have been diagnosed with sarcopenia or osteoporosis, you want to be extra cautious with preserving muscle mass.
